The government is actually responsible because they run the whole operation. The problem is that the government doesn't pay enough money to improve the technology required in order to make the shuttles extremely perfect. They spend too much of it on medical research. They have a lot of money, so why not use it wisely towards space exploration? Learning about space could make people smarter because they then could figure out ways to improve everything around them.
Private industries don't care about safety because they want a good reputation.
It seems like the government cares less about safety than the private industries. Look at what happened to the shuttle Challenger. On January 28, 1986, it exploded 73 seconds after launch, killed seven crew members, and it was all the government's fault. Why? The hard workers and engineers that put a lot of their time into Challenger DO NOT get enough money to make the shuttles as accurate as possible. The government is also raising taxes just so they can get more money to keep for themselves. The government is too greedy and keeps most of the money for THEMSELVES, instead of helping the world and economy. They need to commit completely to the success and safety of the programs. That is just one of many examples that proves the government doesn't care about safety.
